
Paper Work
In order to play any sport at Ridge, including open gyms, conditioning, and try-outs, the athlete must be registered on the Final Forms website. In addition to Final Forms, ALL interested students must have a current OHSAA Physical on file in the front office. A doctor must sign this form which can be printed from Final Forms once registration is complete. You can turn your physical into your coach at the first workout or the office if open.

Athletic Fee Information
Participation fees are $150 for all sports
• Payment of fees does not ensure playing time.
• Students will not be permitted to practice or participate in contests until fees are paid in full.
• A Family Cap is available for junior high and high school students participating in extracurricular athletics and marching band.
*Fees may be paid with cash or check or you may pay electronically with EZpay. Processing fees are non-refundable.
